A village GP surgery could close as a consultation starts on the impact its closure would have on the village.
The building at Hadleigh Boxford Group Practice's site in Church Street, Boxford, needs "significant investment" to meet the standards of a modern GP practice.
Patients are now being invited to take part in a consultation to consider the future of the surgery.
Under the proposals, GP consultations and blood tests would no longer be done from Boxford and the building would close.
All services would move into one building at the sister surgery in Hadleigh.
A spokesperson said: "Over recent years the number of appointments offered at the Boxford branch surgery has decreased with the majority of clinics and services being provided from Hadleigh surgery.
"We currently offer a limited service at Boxford with four basic GP sessions and two blood clinics each week."
Patients are now able to complete the survey online.
The closing date for feedback is Sunday, October 27.
